INFORMED CONSENT FOR HEALTH DATA PROCESSING

By clicking "I Agree" or using MAI PLEXOS, you provide explicit consent for the data processing described below. Please read this form carefully before consenting.

1. HEALTH DATA COLLECTION AND PROCESSING

I consent to MAI PLEXOS collecting and processing my health-related data, including:

Pain Information

Pain levels (0-10 scale)
Pain locations and descriptions
Pain triggers (weather, activity, stress, etc.)
Pain patterns and duration

Mood and Mental Health

Mood scores and emotional states
Stress levels
Mental health notes and observations

Medication Data

Medication names and dosages
Schedule and timing
Reminders and notifications
Adherence tracking

Meditation and Wellness

Meditation session completion
Relaxation and calming status
Wellness activity tracking
Progress streaks and patterns

Vital Signs (via Apple Health, if enabled)

Heart rate and heart rate variability
Blood pressure
Body temperature
Respiratory rate

Activity Data (via Apple Health, if enabled)

Steps and distance
Exercise minutes and type
Active calories
Stand hours

Sleep Data (via Apple Health, if enabled)

Sleep duration
Sleep quality metrics
Sleep patterns and stages

Body Measurements (if provided)

Weight
BMI
Other body measurements

2. PURPOSE OF PROCESSING

I understand my data will be processed for:

Tracking my health metrics over time
Displaying trends, patterns, and correlations
Generating personalized health insights
Providing medication and wellness reminders
Tracking meditation progress and providing recommendations
Creating health reports for my personal use or to share with healthcare providers
Analyzing correlations between different health factors (e.g., pain and sleep)
Improving my health management experience
Providing subscription services (Premium features)

3. AI PROCESSING

I consent to AI analysis of my health data for:

Generating personalized insights based on my data patterns
Identifying trends and correlations
Providing health suggestions (understood to be informational, not medical advice)
Answering questions about my health data within the App

I understand that:

AI processing occurs locally on my device unless otherwise clearly disclosed
If any AI features require sending data to third-party services, I will be informed and asked for explicit consent before any such sharing
AI suggestions and insights are NOT medical advice
I should always consult qualified healthcare providers for medical decisions
AI cannot diagnose conditions or prescribe treatments

4. APPLE HEALTH INTEGRATION

I consent to connect MAI PLEXOS with Apple Health to:

Import health and fitness data
Sync activity metrics
Access sleep information
Share relevant data between apps (under my control)

I understand that:

Apple Health integration is optional
I can enable or disable this integration at any time in Settings
Apple's privacy terms govern Apple Health data
I control which data categories are shared

5. NOTIFICATION PERMISSIONS

I consent to receive notifications for:

Medication reminders
Daily logging prompts
Meditation session reminders
Health insights and milestones
Subscription status updates
App updates and new features

I understand that:

Notifications are optional
I can manage notification preferences in Settings
I can disable notifications through iOS Settings at any time

6. DATA STORAGE AND SECURITY

I understand that:

My health data is stored locally on my device
Data is encrypted using AES-256 encryption
I control all data sharing and exports
No health data is stored on company servers
If I enable iCloud backup, Apple's encryption protects my data
I can delete my data at any time through App settings
Uninstalling the App removes all local data

7. DATA SHARING

I understand that my data:

Will NOT be sold to third parties under any circumstances
Will NOT be used for advertising or marketing targeting
Will NOT be shared with third parties without my explicit consent
May be shared only when I explicitly export reports (PDF/CSV) for my own use
May be shared with Apple for subscription management (anonymized identifiers only)
May be disclosed if legally required by valid court order or law

8. SUBSCRIPTION CONSENT

If I choose to subscribe to MAI PLEXOS Premium:

I acknowledge and agree that:

Monthly Subscription: $6.99 USD per month, billed monthly
Annual Subscription: $41.99 USD per year, billed annually
Payment will be charged to my Apple ID account at confirmation of purchase
Subscription automatically renews unless canceled at least 24 hours before the end of the current period
My account will be charged for renewal within 24 hours prior to the end of the current period
I can manage and cancel subscriptions in my Apple ID Account Settings
Any unused portion of a free trial will be forfeited when I purchase a subscription
No refunds for partial subscription periods; I retain access until the period ends

9. CONSENT WITHDRAWAL

I understand that I can:

Withdraw consent at any time
Delete all my data through App settings (Settings > Account > Delete Account)
Opt-out of specific features individually
Disable Apple Health integration
Cancel my subscription through Apple ID Settings
Uninstall the App to remove all local data

Withdrawal of consent will not affect the lawfulness of processing based on consent before withdrawal.

10. ACKNOWLEDGMENTS

By using MAI PLEXOS, I acknowledge that:

I am at least 13 years old (or have parental consent if required)
I have read and understood the Privacy Policy
I have read and understood the Terms of Service
I have read and understood the Health Disclaimer
I have read and understood the End User License Agreement (EULA)
The App is NOT a medical device
The App does NOT provide medical advice
I should NOT use the App for medical emergencies
I should consult qualified healthcare professionals for medical advice
I am responsible for the accuracy of data I enter
I am responsible for my own medication adherence
AI insights are informational only, not medical diagnoses
Auto-renewable subscriptions renew automatically unless canceled

11. SPECIAL CONSIDERATIONS

For EU/EEA Residents (GDPR)

This consent complies with GDPR Article 7 (Conditions for consent)
Processing of health data is based on explicit consent under Article 9(2)(a)
You have all rights under GDPR Articles 15-22 (access, rectification, erasure, restriction, portability, objection)
You can lodge complaints with your local supervisory authority or the Estonian Data Protection Inspectorate (aki.ee)

12. VOLUNTARY PARTICIPATION

Your use of MAI PLEXOS is entirely voluntary. You may:

Choose not to use certain features
Skip optional data fields
Use the Free Tier without subscribing
Disable optional integrations (Apple Health, notifications)
Delete your account at any time
